Advancing Patient-Centered Care for Advanced Illness

There’s good news coming out this week: C-TAC is ready for prime time.

Healthwise is part of a group of organizations and individuals called C-TAC, the Coalition to Transform Advanced Care, which is calling for reforms in the way care is delivered for advanced illness. Advanced illness occurs when “one or more conditions become serious enough that general health and function decline, and treatments begin to lose their impact. This is a process that continues to the end of life.”

We recognize that all too often, patients and their caregivers don’t get a chance to be heard when it comes to their choices for care during advanced illness. They are not given help and support for decision making that reflects their preferences and values. We can do better. As C-TAC states, “People already burdened with advanced illness should not have to work so hard to receive the quality, coordinated care they need.”

The organizations and individuals that make up C-TAC are working to inform, educate, and empower patients, family caregivers and health care professionals to ensure that the right care is provided at the right time and place. The aim? To give patients and families living with serious illness a voice when addressing their own care.
